THRIVE Q Questionnaire

I recently sent this out to my Q Customers and Team. It has helped me to identify customers who are having problems and promotes conversation with those who are not active on their Q.

THRIVE Q Customer Satisfaction Questionnaire:

1.  The quality of food I receive is:
